Create a excel form with combo and option boxes

Greetings,
I have an Excel 2007 document I recently created.
I have several lines represent questions.  I have created a list box to represt selection 'yes' or 'no'.  I also have a check box.
I would like to create this as a form where users can save a copy and can ONLY update the control boxes.
Is there an easy way to do this? I would like to keep it in excel since word gave me a headache.
